Compensation: $43K-56K annually
Job Overview: Join our team as a Community Care LVN, providing essential health services and support through home, hospital, facility, office visits, and telephonic outreach. You will conduct basic health assessments, assist families with their healthcare needs, provide information, and make appropriate referrals. This role requires building trusting relationships with our family members and caregivers, ensuring comprehensive and coordinated care.
Key Responsibilities:
- Conduct home visits to perform physical exams under remote supervision.
- Provide follow-ups, motivational interviewing, and goal-setting.
- Manage clinical supplies and facilitate telehealth visits.
- Maintain consistent communication with the clinical team and update electronic medical records.
- Act as an advocate and liaison between family members and community service agencies.
Qualifications:
- Licensed Vocational Nurse from a US accredited facility.
- At least one year in a health services environment.
- Community Health Worker experience preferred.
- Excellent communication skills and proficiency in Microsoft Office applications.
- Reliable transportation for frequent travel to homes, hospitals, and facilities.
Physical Demands
- Typical office environment tasks with frequent travel by car or public transportation.
- Ability to carry medical supplies and equipment.
