- Location: Phoenix, Arizona 85009 (where the heat is high, but your impact is even higher)
- Compensation: $60,000-$80,000 annually
Qualifications:
- LMSW license—a symbol of your dedication
- Master's degree in Social Work
- Minimum 1 year of clinical social work experience—a testament to your commitment
- Problem-solving skills with a positive attitude
- Mission-driven attitude—a crucial trait for changing lives for seniors
- PC proficiency
- Fluency in Spanish or other languages spoken by people in the communities we serve

Your Daily Adventures:
- Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each member
- Plan, coordinate, manage and implement support packages to help members deal with social economic and medical barriers
- Navigate managed care plans for community services and programs
- Empower members to take charge of their health journeys
- Document your impact with meticulous detail in clinical management systems
- Guide members and families through advance care transitions with compassion
- Unlock state-based prescription programs and other benefits to enrich our members' lives
