We're on a quest to find a Nurse Practitioner extraordinaire to join our client's dream team in Syracuse, New York (zip code 13244, for those who navigate by Orangemen basketball courts).

The Scoop:

  • Hybrid Role
  • Schedule: Monday-Friday, 8am-4pm (with some on-call rotations)
  • Compensation: Up to $120k

Your Superhero Toolkit Should Include:
  • A shiny New York NP license
  • NPI, Medicaid/Medicare, and CAQH numbers
  • Spanish-speaking skills ideal (porque la salud no tiene barreras lingüísticas)
  • Experience in home health (because not all heroes work in hospitals)

Your Mission Responsibilities, Should You Choose to Accept It:
As our medical maestro, you'll:
  • Deliver comprehensive medical care across diverse patient demographics, including adults, seniors, and children.
  • Conduct thorough evaluations, encompassing detailed history-taking and physical examinations to assess risk and comply with regulatory standards.
  • Address both chronic and acute primary care issues, adeptly assessing medical urgency.
  • Order necessary diagnostic tests—laboratory and radiology—judiciously and effectively within resource constraints.
  • Participate in 24/7 on-call rotations to ensure continuous patient care availability.
  • Develop and implement personalized medical care plans tailored to patient needs.
  • Coordinate care and provide preventive services for both in-person and virtual patients, stratified by risk factors.
  • Facilitate post-discharge coordination to minimize hospital readmissions and emergency room visits.
  • Conduct in-person visits across various settings to meet patient needs.
  • Utilize Tele-Health platforms for synchronous video communications, following organizational directives.
  • Prescribe medications as deemed appropriate based on patient assessments.
  • Maintain accurate and timely documentation in electronic medical records.
  • Practice medicine collaboratively under the supervision of licensed physicians, adhering to state regulations and guidelines.

You'll need to be ready for 24/7 on-call rotations (because villains... er, illnesses don't sleep), and you'll be working your magic in homes, clinics, and even through screens. Your documentation skills should be sharper than a Syracuse winter wind, and you'll be collaborating with physicians like Batman and Robin (minus the capes).
