Position Description:
The Practice Transformation Facilitator (PTF) reports to the Practice Transformation Manager, providing strategy, judgment, organization, and evidence-based analysis to influence decisions and meet requirements. Primary Responsibilities:
Manage a team of Practice Performance Facilitators across various practice settings.
Optimize information technology resources to enhance patient outcomes and office efficiencies.
Collaborate with interdisciplinary teams to support healthcare practices, including population health management, care coordination, and patient action plans.
Serve as the primary liaison between healthcare organizations and external stakeholders.
Guide care teams in adopting best practices and implementing quality improvement strategies.
Provide training on technology tools and ensure compliance with regulatory guidelines.
General Duties:
Lead and execute strategies to create business value in clinical areas.
Establish business strategy and roadmap for improving outcomes.
Collaborate with other business divisions to define solutions.
Provide subject matter expertise in clinical solutions.
Create a productive, collaborative, and inclusive work environment.
Qualifications:
Education: Bachelor’s or equivalent degree with strong academic achievement.
Experience: At least 3 years of clinical practice experience. Ideal candidates will have 5+ years in healthcare with successful implementation of quality improvement initiatives.
Skills: Excellent communication, relationship management, and team collaboration.
Location: Venice Beach, CA
Compensation: $66,000 - $70,000 annually
